Now that's a good point. The American medical system does consider it "viable" due to the generous fees they charge for it and the public's acceptance, whilst your "private" specialist in Great Britain likewise considered it "viable" according to the fee he charged, but then he has developed a "fringe" market which is essentially, cosmetic surgery.
In other countries such as Australia, the market is similarly limited by the demand which continues to shrink, even if slowly, and in Europe, the market is negligible.
Practitioners willing to do it on unconsenting children will be much fewer as they consider the risk of future litigation for aggravated assault - this is getting to be a hot topic in Australia.
