This happens to me too, though more often when I'm soft than when I'm hard. It doesn't track correctly as it rolls forwards, and the skin that should end up at the tip instead ends up rolled up inside. This doesn't seem to be much to worry about, although my own foreskin definitely prefers to sit in a particular way.

If it happen when I'm soft, I just tug it forwards, which sorts it out, though you should probably watch what you're doing here as this could cause painful friction as the inner foreskin pulls over rather than rolls over the glans.

Occasionally when I'm wanking something like this will happen too (it never seems to happen when there's enough lubrication about). What seems to happen is that the skin rolls unevenly. Skin on one side rolls too early, while skin on the other side rolls too late. Skin that is accustomed to roll on a single, synchronous front is now rolling at different times. It becomes kind of twisted. If this happens, I retract it again, stretch it back a bit, and then roll it forwards again. This always seems to sort it out.

Lubrication seems to reduce the liklihood that this will happen almost to zero. It seems to me that the skin doesn't really like being twisted like this, so you should probably go at it gently and maybe use a fairly gentle grip until it is well practised at rolling.

Designing a double-layered sleeve that will roll back on itself, turn itself inside out, and then roll forwards again without snarling up is no mean feat. It's very, very clever in fact. The foreskin is a tremendous piece of engineering: a kind of 3D caterpillar track. To work reliably it may need some interesting topological properties (such as a frenulum-style line of attachment) that no-one has thought to investigate yet.
