It sounds like you've been making impressive progress: from no retraction when soft to full retraction even when hard in only 4 months. This is good going.

If the hospital appointment is only for an initial consultation, then you shouldn't have anything to loose by keeping it. You might even get some useful information. If the appointment is for surgery, on the other hand, well...

The progress you've made so far suggests further progress is possible, and that given a little more time, you will have a loose, easy-rolling foreskin. This, I reckon, is ideal rather than completely normal. It's like perfectly even bright white teeth with no fillings: ideal rather than average.

If you keep stretching for a little longer - another four months, though maybe you'll manage it in two or less - you should find your foreskin really starts to loosen up. With practice, and given the opportunity, you'll be able to pump fast and hard, but you'll also be able to take things really, really slowly.

You can retract your foreskin when you're hard, and can presumably roll it forwards again. So, if you can find about fifteen minutes a day to work on this, with normal luck you should find it develops a loose, silky action pretty quickly.

It sounds like you're at Stage 2 or Stage 3 of the exercise program described in Stretching Exercises. Why not give this program a couple of months before you try anything more radical?

Porn films, even European ones, tend to show only a very pumped up, thrusting style of sex. But this isn't the only style of sex there is. There is slow sex too, where the object is to keep things at a simmer, and stimulate all the erogenous parts of your partner's body with a very light touch, so that your partner slowly begins to melt. You melt too of course.

With practice you can keep this up for an hour or more: slow, highly erotic sex, teetering on the brink of orgasm. This sort of sex is seldom shown on porn films because, well, although it feels great, unless you are one of the people involved, it probably looks rather boring, and anyway, it can last at least as long as most porn films do. Sex in this style can be extraordinarily intimate and loving, and isn't merely a technically accomplished mechanical performance.

It seems to me that in slow sex, the sensations produced as the foreskin rolls are what keep you hard. With me at any rate, it's these sensations that make it possible to go very slowly - tai chi sex - but still stay hard. Now it could be that I'm wrong about the foreskin being essential to this, and that circumcised men can go slow too, and can stay hard for an hour or more, and can be just as tai chi as intact men. All the same, this isn't a risk I'd be prepared to take.

Another thing. Presumably the guy in the porn film was ripped. He was lean, and had a six-pack. Well guess what: almost any normal young guy can be ripped. It only takes a bit of application. Regular work in the gym. Good diet. Running, rowing, swimming... And it's much the same with your dick. With regular exercise, it will begin to work amazingly well. You'll love it. You will be proud, and will want to share it with an appreciative, significant other. I don't know why exactly, but this simple piece of information - that good sex requires training - is almost entirely hushed up by our culture. (I dislike this, and also dislike various quick-fix surgical solutions that seem to me to help perpetuate the hush up.)
