Interesting combination - I might just point out for completeness, that this is not actually hydrocortisone, but rather one of the stronger steroids, and whilst the inclusion of nystatin, an anti-"thrush" agent, is good (as these problems with fissuring are most likely to be caused by Candida/ "thrush"), the other antibiotic is in most cases unnecessary and it is not a good idea to use antibiotics without a good indication.
Essentially, the combination resembles the well-known "Kenacomb" and should not be used for a longer term, but other combinations would be quite a bit safer.
