What Paul has outlined is exactly what I was thinking when I read your description. Because your opening is so tight, it would be difficult for you to get a grip on the skin in order to stretch it. Using a device such as the cap of pen, as Paul has suggested, will provide the tension you need to relax the skin cells. I caution you to use this only during waking hours, not allowing the device to remain inside for several hours without removing it. Also be careful during insertion not to guide it into the meatus (pee hole).

As the skin cells relax and muliply, you'll eventually be able to move up to a larger size device, or you can start manual stretching.

Meanwhile, while peeing, pull the skin back each time. By pulling it back to its greatest limit and holding it there, you are also creating tension.

With the two exercises used together, you should see some favorable results in a very short time. Remember, it is not necessary to create much tension at all to see these result. Gentle, constant tension will get you to your goal much faster than applying too much stress upon the tissue. Do you have any quesitons about how to go about this?
