How far back? Well, as far back as you can tolerate with the slightest of discomfort. I think most people should have the common sense to determine when it's enough, and short of tearing. The risk of tearing is more when you "let yourself go" during sex, which may equally include masturbation or intercourse.
Your first goal is as Ralesk says, to expose the glans completely, with clearance all the way around, and to ensure there are no adhesions (which can be slowly pulled loose). The second goal is that the fraenulum stretches enough that the glans is not pulled down when the foreskin is pulled back to expose the head while the third goal, much less important, is to have the foreskin cover the shaft evenly.
How long to hold? A minute or so, perhaps more. If you have the time, a few minutes, perhaps while you are studying, browsing 'net or whatever else you do in sufficient privacy.
How often? Well, that is easy - as often as you have the chance. Stretching more often for short times, is probably better than a longer time, once only. But the "chance" to do it has some bearing also - the combination of privacy and an erection to some degree is also necessary.
A good example of the combination is when you go to pee, so as previously explained somewhere here, you most certainly pull it back for either a few moments, or a minute or so, whenever you go to pee, before you do so. It may not be practical because of the risk of "spraying" (which is however, less problem in the shower) to stretch back while you pee, but it is an eventual goal that you will do this and that is in the final result, a means of ensuring it stays fully mobile.
