It doesn't have to. Use some lubrication so that your foreskin is not pushed back but slips right in. This is no different than a cut boy has to do. You don't need a prescription for it, KY is available everwhere or if you have sex shoppes where you are, they will have lube like Astroglide. You should be using a condom for penetrative sex anyway. Roll the condom on your penis, foreskin forward, and apply latex safe lubricant to the outside of the condom, something that always needs to be done when using condoms.

Stretching will work in almost every case, but it must be done on a regular basis. You may need to use some Betamethazone ointment or cream to help your foreskin start the stretching process.

I suspect you are talking about Betamethazone, not lubrication. Lubrication certainly doesn't require a prescription. Yes, you will need to see a doctor to get a prescription for Betamethazone. Make an appointment to see your GP, tell him you are tight and want to use the cream to stretch. If he wants to circumcise you, tell him you want to try the Betamethazone first, it can't hurt anything. The doctor normally isn't going to be excessively interested in your dick, he may ask to see it, that will confirm that it's tight and that will be the extent of it. The doctor will probably cost you an office visit since you don't have socialized medicine or your insrance will cover it if you have it. Betamethazone is verey inexpensive.
