Paul, is the betamethasone of any use at this point? My thinking is that it may cause the skin to thin out which might hinder the separation process. If there is thrush as you have suggested, would the use of it not be contraindicated? You have suggested also that a short frenulum would have prevented the normal separation, but the subject has stated that it was previously separated, only lately becoming stuck.
