And indeed that is the nub of it - the fraenulum is not really the problem. As you have postulated - to what does the fraenulum attach? To the foreskin of course, there is nowhere else it could. That is after all why (some of) those who have had a circumcision feel the need to have further surgery on the fraenulum - having had the lower part of the foreskin removed and the remnants sewn together, the attachment of the fraenulum is foreshortened to the extent that it becomes a problem on erection that it was not previously.

Erections, notably nocturnal ones, generally guarantee that both the foreskin and fraenulum stretch to accommodate; where the foreskin does not retract, then it stretches to fit in the un-retracted state and a short fraenulum is only then noted on attempts to retract the foreskin.

Exactly. Stretch the foreskin sufficiently, and the attachment of the fraenulum no longer matters.
