Yes, that certainly is an important detail. By implication, the diabetes has been present for at least ten years and is the most likely cause of the phimosis (through the development of chronic candidiasis).
But it is hard to discern what you are asking - what answer you expect? Most obviously, if you suspect you have a urinary infection, you need to report this to the doctor and have a mid-stream urine collection for culture. You will be unable to comply with the usual pre-requisite - that you retract your foreskin before you take the sample, but it will suffice that you make it a true "mid"-stream sample, by waiting until you are peeing at maximum force before, and continue to do so as you put the specimen container into the stream to collect the specimen, and move it out again of the stream without stopping.
If you have a UTI (Urinary Tract Infection) to which you are predisposed by diabetes, then that is but one more of the complications of that condition with which you will need to deal. If you do not have a UTI, then the irritation may be a more active condition of the "thrush" which I presume to have caused your phimosis. This can however be treated either by squirting a suitable anti-thrush cream through the narrowed opening of the foreskin and massaging it around your penis head - there is nothing odd about this - that is exactly what women do to treat the same condition - or you can take an oral medication (from the doctor) for this.
Your diabetes is obviously the matter that is of direst import to your health, but you might care to point how the phimosis affects you? What effects has it had on your sexual enjoyment (itself at substantial risk from the diabetes) and how would you wish to remedy it? And of course, how old are you?
Another small clarification - you say urination is uncomfortable whilst flaccid - are you implying it is not so whilst erect, not possible, or simply not something you have verified?
