The needle is certainly quite thin to be sure, but there is a little bit of poking and probing about with it to find the right place (as I said, somewhat less poking and fiddling for a spinal than an epidural), so that may provoke a bit of bleeding.
So if there's a lump, it will generally be due to a bruise (haematoma) from said "probing". Most of the guys use a bigger needle first to make a hole in the skin so the fine one isn't blunted by going through the skin.
"Horrific"? Well, I suppose if you get upset by such things!
