Yes I realize that now, but tell that to my 14 year old self, that used to be a real worry, I didnt know how long theyd have to play with it to see what problems I had with it. I was also embarrassed about masturbation back then, and I used to worry they could somehow find out I did that and would know. Agian unrational, but those are the fears of younger boys. Or atleast for me, and for some people I have talked to have said they sometimes feared the same things. And actually I have had a female doctor give me a physical once, and it was a very nervous feeling for me because I just knew that if she cupped me and made me cough that I would become turned on, but I suppose they have diffeent methods if it is a female, and she just pressed against my lower stomach.
So in most cases I would say they take into account that younger kids are hormonal, but the point is to put the kids fears to rest, so that he will go for the help that he might need. If he will let his fears keep him away, then that is a method to give him a little reassurance. Whether it matters or not isnt the problem, its getting him confident or assured just enough to get him to get the help he might need.
And you docotrs dont give you hand-jobs? Poor you!!
