Let's suppose the urologist is a civil servant getting paid the same no matter how many operations he does or does not perform, just handling whatever walks in the door. He is presented a case where the GP has recommended cutting the foreskin off once and for all. The civil servant can either handle the case in under an hour and be done with it, or have to see the fellow weekly or so for months. Either one counts as a resolved case. He clearly has a motivation to cut, and mark one more case "cured" for his performance review. But of course it is not cured any more than any other amputation - the immediate problem may be gone, but others arise due to the unnecessary and drastic response.
