Phimosis?

It seems like i have a kind of phimosis because, especially when it is erect, my foreskin wont go back and has a small hole that wont let the penis come out.. when its normal it is a little bet hard but it goes back ... What do i need to fix this ? is surgery the only solution ? Can someone pls tell me something that might help ? Pls answer here my email doesnt work
