By the way, an adult man whose foreskin resists retraction because the diameter of the preputial orifice is narrow, can remedy the situation IF the man wants it to be remedied...by a simple regime of daily manual massage and stretching. Penile skin is highly elastic and responsive to stretching. Stetching is free, too. Doesn't cost a cent.

PS: "Phismos" is not a word. We must assume that you meant to write "phimosis", which is not a medical condition requiring attention...it's merely a name given to an unretractable foreskin, a condition which is not ideal, but which seldom if ever requires the use of scalpels.
