Well, with any common sense, it's highly impossible that the plain presence of the foreskin would be the cause of all these, so I do not agree with the urologist's suggestion. Not to mention that the exposed glans would be even more prone of getting irritated, for a good while (especially if it's now inflamed, it won't be anything nice for the child).

If you use soap or soap-containing substances around his privates, cease with the use. They might be one of the reasons for the irritation. Cleaning with clean water is more than enough. Not sure, but sounds logical, that the discharge is better to be washed away, too. (Paul B, anyone?)
Since you mentioned 2-3 times a year, it means he is without problems in the rest of the time. When this is the case, it might be a good idea to follow one of the manual (device-free) skin expansion techniques that will solve phimosis. The only time they fail is when they're done improperly. The resulting retractable foreskin will provide normal sexual function.
