It's not going to be as bloody embarrasing as you think. Believe me, your dad will well understand, he's got a willy too, you know.
Be calm. He'll be impressed that you've thought it through. He may ask you to describe what's wrong. No need to go into a detailed life's history, just tell him you were doing your best to keep clean and the soap's given you an irritation and you want something from the GP to clear it up.
The GP's going to recognize what the problem is, he/she's seen it before, on boys and girls alike, so she's not going to tramp through the surgery hollaring about the rash on your willy. It will all be quite private, as it should be.
As I mentioned, best not to wank yourself till everything's settled down. That won't be long though.
