It seems that the problem is that you went to a plumber, when you needed a siding man. That is, if you had a skin infection, you should have gone to a dermatologist rather than a urologist. Just because the infection is inside your shorts does not mean that the urologist is the doc for the job. Please address the cause of the tightness first. Once that is resolved, then you can give appropriate creams and stretching a fair shake before taking an irreversible step.
