Did the doctor take any tests to determine exactly what the problem is, or is he just guessing? In order to treat something right, it is necessary to know what is being treated.
Whatever you do, don't let that urorologist talk you into cutting of any kind on the boy's penis. The foreskin didn't cause the problem, so there is no need for "getting even" with it. Once the problem is properly diagnosed, we can help you in getting him to stretch the foreskin so it operates correctly. While at age ten, it is not necessary for it to retract, it might help if he were to learn early.
Please answer Paul's questions so we can get a little better idea of the condition of the skin. The balloning issue is a critical bit of information.
