From what you've said, other than having a tight foreskin, there's nothing wrong with his sheath, indeed, this pain is a recent thing, the tight foreskin didn't cause him bother before.
I will gaurantee you that the first thing the urologist will want to do is circumcise him, and afterwards, you're son will be minus his foreskin and the urologist will say, well that wasn;t it, but it needed to be done anyway.
You indicated that they did a urinalisis and that it came back negative. That should have zhowed any bladder infection or UTI.
I would suggest that you have your son tested for diabetes. That is a classic symptom of diabetes along with constant thirst, weight loss, etc. The test is simple and painless. In fact, if you know anyone who is a dibetic, ask them to do a test on his blood sugar. All it involves is a tiny pin prick and your friend will be able to instantly (30 secs) tell you if it's high.
That's my best guess. I've been there and the burning sensation as you pee and even after is extreme.
