Your girlfriend must be told what feels great, and what doesn't, just as she must somehow indicate to you her reaction to your efforts to pleasure her...if only through groaning and moaning. Whatever the case, she can be sure that when you reach that point of "pain" that she has been completely successful in her efforts. Let her know by thanking her afterwards.
